Service Description
Take a deep dive into your health using a Lifestyle Medicine lens! Lifestyle Medicine seeks to address the root causes of disease. Good health is a matter of balance. We must achieve balance in many areas of our bodies to live our best lives. To provide this Deep Dive Consultation, we require an extensive health history questionnaire, intake form, and 5-Day food journal be submitted prior to the scheduled consult. Any labs that you have can also be submitted. Even things that don't seem significant can be a clue to what is causing the symptoms of disease. We will use all of these clues to take a deeper look at your health and provide a plan specific to you and your health and wellness goals.
Cancellation Policy
The Client understands the cost of this Program is $249 and is due in full at least 5 days in advance of the DEEP DIVE Session. The Client will pay for the Program by bank account withdrawal, Health Savings account, or credit card through the online payment portal. In the event of the Client’s absence or withdrawal, for any reason, the Client will remain fully responsible for the full price of the program due to the upfront investment in developing a specific program for the Client. Special circumstances will be considered on a case-by-case basis. Additional services beyond what is included in The Program will be charged as follows for each incidence: Additional Laboratory Review $79: Medication Therapy Management Review (beyond initial consultation) $99; Follow-up Consultation (beyond the two included in The Program) $99; Additional Check-In Call $49; Additional Quick Call $29
